4.1 Complete the Hiring Documentation
Once the position is accepted, it is the responsibility of the hiring department to prepare the employment package for the successful candidate and submit the required information to HR as quickly as possible to facilitate the processing of the new employee’s first paycheck.
For faculty appointments, the department should ensure that the successful candidate has returned a signed copy of the offer of employment (signifying their acceptance of the offer), to the Vice Provost. The department should also ensure that the new faculty member makes an appointment to meet with a staff member from the Benefits Office in order to complete the required forms to be placed on salary and to be enrolled in the applicable benefits plans.
The following documentation should be completed and forwarded to HR as outlined:
Formal letter of offer (2 copies)
Candidate to return one signed copyOriginal resume of successful candidate
A completed and signed Personal Information Form (PIF) if the candidate is not already an employee of the University
A TD 1 if the applicant is not already an employee of the University (will open in a new window)
Appropriate authorization if the individual is not a Canadian citizen
Direct Deposit Form if new hire or new request for direct deposit (will open in a new window)
