College/Departments will be required to download, print and complete the OMR Processing Request Form for Data
Each College/Department is responsible for separating the response form batches by using a "Batch Header Sheet" per batch. These forms can be obtained from the ICT Help Desk (70 Arts, 966-4817) at no cost.
Once the Course Evaluation is ready for processing, the College/Department must:
to the ICT Help Desk located at 70 Arts Building. Course Evaluations can be received during hours of operation. Course Evaluations, excluding SEEQ paper-based evaluations, will be processed within 24 to 48 hours of receiving them. The actual time to process will depend on the time of year and demand on the service.
SEEQ paper-based forms can be purchased directly from the ICT Help Desk (70 Arts) at a cost of $0.20 per sheet. OMR forms must be printed on particular OMR paper stock, which has calibration marks for the scanner. Photocopies of these forms do not scan.
Requests to modify the standard SEEQ paper-based form be should be directed to seeqhelp@usask.ca.
The paper-based SEEQ evaluations will be processed after the last day for submitting final marks for the term in which the evaluations were completed.
The OMR Processing Service supports a general course evaluation form that is available from Supreme Basics. Colleges and Departments are responsible for purchasing these forms directly from Supreme Basics using the product code "OPSCAN" for U of S.

The ICT OMR service can scan OMR forms to produce a raw-data file of the scanned responses.
The raw-data file of the scanned responses is a tab-delimited file that contains the responses as captured directly from the OMR form. This file has not been altered by any post-processing procedures and is available to clients who choose to perform their own computing of results.
The scanning service is provided free of charge for all U of S academic and business use.
Standard course evaluation reports will be produced for free when using the SEEQ paper-based or the general course evaluation forms. Custom reports can be produced at a cost. Please contact the ICT OMR Service to request a quote.
