Please use the following form to request a Fluid Surveys account. Note that you will need your University of Saskatchewan NSID (Network Services ID).
When your account is ready, you will receive an email which you can use to complete your registration. Note that the password that you create can be anything. It is NOT coupled with your University of Saskatchewan NSID. If you forget your Fluid Surveys password, please contact a Fluid Surveys administrator to have it reset.
Note that you will always need to use your NSID email address (something like "abc123@mail.usask.ca) everytime you logon to Fluid Surveys. Other email aliases will not work.
