The University of Saskatchewan (the "University") provides services for developing and hosting online surveys (the "Service"). The Service is provided to users of the Service ("you" or "Users") subject to the terms and conditions set forth in these Terms of Use ("Terms of Use" or "Agreement") and any other rules or policies stipulated by the University from time to time. Please read the following terms of this Agreement carefully. By using the Service‚ you accept and agree to all of the terms and conditions set forth in this Agreement. You hereby acknowledge that you are 18 years of age or older.
- The Service is provided by the University on an "as is" basis‚ and the University expressly disclaims any and all warranties expressed or implied including without limitation any warranties for fitness for a particular purpose. In no event shall the University be liable for any direct‚ indirect‚ incidental‚ punitive or consequential damages whatsoever with respect to this Service. It is the responsibility of the people conducting surveys to evaluate this software and decide if it is appropriate for their use.
- You acknowledge and agree we may‚ with or without notice‚ modify or terminate the Service. The University is authorized‚ but is not required to‚ monitor usage and activities within the site and the University has the right to remove‚ classify‚ revise or censor content from the site without notice at its sole discretion for any reason whatsoever.
- The Service is offered as a community Service for the University faculty‚ staff‚ and students so they can conduct online surveys. People conducting surveys using this tool personally accept full legal responsibility for their surveys. This includes but is not limited to legal responsibility for the wording of questions on the survey‚ procedures used for conducting the survey‚ and appropriate security of data collected by the survey. The University and Information and Communications Technology (ICT) will provide the Service‚ but does not accept responsibility for how individuals use the Service.
- Users must abide by all University policies. These can be viewed at http://www.usask.ca/university_secretary/policies/. This includes but is not limited to policies related to computer use‚ copyright‚ data use‚ and confidentiality of information.
- Users must abide by the laws of the Province of Saskatchewan and Canada. This includes but is not limited to laws governing the collection and handling of private data and confidential information.
- Users agree to never ask participants for the following information:
- Credit card or bank card numbers.
- Passwords to any Service‚ on-campus or off-campus.
- Information which participants cannot legally divulge.
- Users agree to protect the confidentiality and privacy of survey participants and the responses provided by survey participants.
- Users agree to obtain all appropriate approvals from the University of Saskatchewan Research Ethics Board and any other appropriate agencies before conducting their survey.
- Users agree the University and Information and Communications Technology (ICT) may divulge survey results and information about usage of this survey tool to authorized legal authorities in order to assist in criminal investigations‚ upon receipt of an appropriate court order.
- You agree to defend‚ indemnify and hold harmless the University from any and all claims and demands‚ including legal costs on a solicitor client basis‚ due to or arising from your use of the Service‚ survey responses‚ survey results and any other conduct related in any way to the Service.
- Before conducting a survey please contact the Office of Institutional Planning. Institutional Planning and Assessment is responsible for maximizing the benefits from surveys on campus by:
- Ensuring good survey methodology and design;
- Encouraging the communication of survey results with the U of S community;
- Avoiding the collection of duplicate information; and
- Reducing survey fatigue by limiting the number and timing of surveys to any one group.
