If you want to create and run an online survey‚ please:

1. Read the features on the Web Survey Tool Home page. It describes what the survey tool can do.
2. Review the Limitations information. This lists known limitations and issues with the survey tool and tells you what it cannot do.
3. Read the Terms of Use document. This document explains the official policies and procedures which you must follow when using this service.
4. Visit the survey Demonstration.
5. Log in to the survey system at the Survey Tool Login page or log in to PAWS and add the My Surveys channel to your layout.
6. Click on "New Survey" to create a survey.

Supporting Your Survey

Survey owners are expected to provide appropriate support for participants in their survey. ICT will not provide assistance in interpreting survey questions or assisting survey participants with problems resulting from the construction of the survey.

Survey owners are responsible for assisting participants with logging into the survey. This is especially true for participants who are not logging into the survey using their Network Services ID. (NSID)

The ICT Help Desk will provide assistance in changing passwords for NSID usernames.
