Outcomes and challenges involved in community development work are also highlighted.

Examples are given which illustate how the principles of primary health care are integrated into community health nursing practice. The role of the community health nurse in implementing primary health care is portrayed, as well as the challenges and outcomes inherent in this process.
- Taking Responsibility to Heal OurselvesDescribes how lifestyle changes over the last four decades have contributed to increases in diabetes among native people. Features what this one community is doing about it. The Kahnawake School Diabetes Prevention Project has developed a model whcih can be implemented in other schools and communities.
- The Challenge of Healthcare LiteracyAs healthcare has become more complex, the ability of a large percentages of the population to understand their own healthcare needs, prescriptions and other medical instructions and make informed choices has not kept pace. The entire healthcare profession must make an effort to offer healthcare information in a manner that makes it comprehensible to all, without creating a sense of shame for those with healthcare literacy problems. All providers must employ strategies that ensure that all information is clearly understood. This program provides healthcare personnel information and techniques to use when facing the challenges and risks of low healthcare literacy, and describes what actions to take to help all patients make informed healthcare choices
